The role of a Community Access Nurse

The Community Access Nurse seeks to assist Members to remain independent and living within their own homes, where possible and practicable, by trying to source community based supports. Community Access Nurses will see people in their homes, at one of our Centres or in the hospital clinics or wards to provide Information about MS and the medications, including immunotherapy. Individual assessments can be done and care plans developed as required.

Education and training on individually prescribed products, injection techniques and ongoing support and advice on managing side effects of the medication are provided.

The nurses make country trips annually to touch base with our Members in regional centres. However if you would like a nurse to speak with you by phone or email just call 08 9365 4888 or use contact us.

The role of a Hospital Liaison Nurse

The MS Hospital Liaison Nurse works with people with MS, including the newly diagnosed, both in the community and the hospital system, in the MS clinics and on the wards. They work closely with the treating teams in the hospital and assist with discharge planning. Please download a HLN service brochure here.
