The Neurodegenerative Conditions Coordinated Care Program (NCCCP) is a new pilot program funded by the Disability Services Commission (DSC) providing supports and services to assist eligible people with rapidly degenerative conditions to remain living at home.

The program, a DSC initiative, seeks to provide timely information and support for people with rapidly degenerating neurological conditions and whose support needs will increase over time. Services include access to a loan pool of equipment and the program nurse.

Referrals for this program may be received from Support Groups, Service Providers, Disability Services Commission, Neurologists, General Practitioners and Self Referral.

People referred for NCCCP services must be eligible for Disability Services Commission funded services, and meet the specific program parameters in place.
